二、申请人的资格要求：

normal style='text-indent:28.0pt;mso-char-indent-count:2.0'>1.满足《中华人民共和国政府采购法》第二十二条规定；

normal style='text-indent:28.0pt;mso-char-indent-count:2.0'>2.落实政府采购政策需满足的资格要求：本项目不专门面向中小微企业、监狱企业、残疾人福利性单位，对符合条件的小型和微型企业、残疾人福利单位、监狱企业给予10%的扣除价格，用扣除后的价格参与评审。

normal style='text-indent:28.0pt;mso-char-indent-count:2.0'>（1）本项目接受联合体投标，且联合体成员不得超过两家单位。

normal style='text-indent:28.0pt;mso-char-indent-count:2.0'>（2）联合体牵头人应具有省级及以上检验检测机构资质认定（CMA）证书（提供证书复印件并加盖公章）。

normal style='text-indent:28.0pt;mso-char-indent-count:2.0'>（3）联合体各成员之间应签订《联合体协议书》。投标人的投标文件及中标后签署的合同书，对联合体的每一成员均有法律约束力。联合体各成员之间签订的联合体协议书应与投标文件一起递交。

normal style='text-indent:28.0pt;mso-char-indent-count:2.0'>（4）参加联合体的各成员不得再以自己名义单独投标，也不得同时参加两个联合体投标。如有违反将取消该联合体及联合体各成员的投标资格。

normal style='text-indent:28.0pt;mso-char-indent-count:2.0'>（5）联合体牵头人作为投标全权代表，代表所有联合体成员负责执行一切与该项目招投标活动有关的事项。

normal style='text-indent:28.0pt;mso-char-indent-count:2.0'>（6）联合体组成后不得再发生变化；中标后，未经采购人书面同意，联合体的成员结构、相互关系等均不得变动。

normal style='text-indent:28.0pt;mso-char-indent-count:2.0'>4. 未被“信用”（www.creditchina.gov.cn）、“政府采购网”（www.ccgp.gov.cn）列入失信被执行人、重大税收违法案件当事人名单、政府采购严重失信行为记录名单。

normal style='text-indent:28.0pt;mso-char-indent-count:2.0'>注：1、凡为本采购项目提供整体设计、规范编制或者项目管理、监理、检测等服务的供应商，不得再参加本采购项目的其他采购活动；

normal style='text-indent:28.0pt;mso-char-indent-count:2.0'>2、单位法定代表人/负责人为同一人或者存在直接控股、管理关系的不同供应商，不得参加同一合同项下的政府采购活动。
