normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>1.针对采购文件内容提出意见和建议的，需明确有意见的采购文件内容所在页次和行数，并说明提出意见的理由和依据；

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>2.潜在供应商须在书面材料上加盖单位公章，注明联系人、联系电话等通讯方式；

normal style='text-indent:24.0pt;mso-char-indent-count:2.0;line-height:150%'>3.有关专家提出意见和建议的，须有本人签字，注明联系电话等通讯方式，同时还应提供本人与该项目相关的专业证书复印件（如政府采购评审专家证书或职称证书、工作证明等）；
